Family Homelessness Prevention and Assistance Program Exit Assessment Form

This is a program exit assessment form developed by Wilder Research for the Family Homelessness Prevention and Assistance Program in Minnesota. The form asks why the client is leaving, where they are going, what type of housing they will be going to, if they will be receiving any subsidies, and their income and source of income at the time of exit. This assessment document might be helpful for creating an agency's own exit assessment documents.
